What to expect
Participants will complete a brief and confidential memory screening with an experienced nurse practitioner and receive:
a summary of their results, which they can share with their existing care team.
education on modifiable dementia risk factors and signs of normal cognitive aging versus potential decline.
information about free and low-cost community-based resources related to brain health and aging.
